Two-wheeled ambulances already operate in several Indian states, typically as first-responder vehicles carrying oxygen, a defibrillator and basic life support equipment to a patient before a conventional ambulance arrives. Their legal position, however, has been unsettled, because the Central Motor Vehicles Rules, 1989 recognise no such category. That gap affects everything downstream: how the vehicle is registered, what equipment it must carry, whether it may use a beacon or siren, what fitness testing applies, and whether it can claim right of way. The draft rules close the gap by placing these vehicles within the L2 category, prescribing a technical standard for their construction, and treating them as transport vehicles subject to periodic fitness certification.

The value of an ambulance lies in how quickly it reaches the patient, and a four-wheeled ambulance is defeated by exactly the conditions where emergencies are hardest to handle — narrow lanes in dense urban settlements, hill roads, and rural tracks that no van can negotiate. A two-wheeler cannot transport a patient, so it does not replace a conventional ambulance; it moves a trained responder and equipment to the scene within the critical first minutes, and stabilisation there is often what determines survival. Leaving the choice of operational areas to state governments is sensible, since the case for these vehicles is strong in the hills of Uttarakhand or the lanes of an old city and weak on a highway corridor. The one-year lead time to October 2027 gives manufacturers time to certify designs against AIS-209.
