The study identifies livelihood constraints, weak awareness, poor grievance responsiveness and inadequate follow-up as the main causes.
For women it additionally found household responsibilities, social hesitation and male dominance during discussions to be barriers.

The two are constantly confused and the distinction is the whole point of the study. The Gram Panchayat is the elected body — a set of representatives chosen for a term. The Gram Sabha is not elected at all: it is the entire body of registered voters of the village, meeting directly. It is therefore the only genuinely direct-democracy institution in the Indian constitutional scheme, and the Gram Panchayat is accountable to it rather than the reverse. That is why low attendance matters so much more than it would at an ordinary meeting: when the Gram Sabha does not meet properly, the Panchayat's plans, budgets and beneficiary lists pass without the scrutiny the Constitution intended, and the accountability mechanism simply does not operate.
Simple Analogy: The Panchayat is the board of directors; the Gram Sabha is the meeting of every shareholder — and it cannot hold the board to account if nobody turns up.
